A 14-year-old male patient with thalassemia intermedia was operated under spinal anaesthesia. Anaesthesia was given at L3_L4 space with Bupivacaine heavy 3ml+butorphanol 0.5mg with spinal needle 25G needle with midline approach at first attempt. At postoperative period there was a moderate pain on left lower limb with only great toe movement, whereas right lower limb movement was normal. The patient was unable to micturate too.The injection site was normal looking.
As no facility of MRI was available we assumed it as acute nerve compression and we started Methylprednisolone, and methyl cobalamine with PPI coverage at the 6th hour of the postoperative period. Bladder catheterisation was done. There was hematuria though preoperative investigations were within normal limit.
