69-year-old male woke up in the morning with weakness in his left leg. He noted difficulty in lifting up his left foot from the floor. He has no other complaints or feeling of illness. The patient generally lead a sedentary lifestyle and spend prolonged period of time with his computer sitting crossed legs. The patient is suffering from Type 2 diabetes and hypertension for past 11 years and his diabetes is presently under control.

He is under lisinopril, diltiazem, insulin 70/30 at present. Physical examination reveals stable vital signs and extremities revealed no c/c/e. What is the likely diagnosis? How can be the first line of treatment?
