The keyword in the above heading is quite obviously ‘continue’—the implication being that in spite of being cognizant and fully aware that it is a mistake, we continue to repeat it. All the mistakes and corrections thereof, and the need to avoid them may be already known to you, having been stressed in some medico-legal seminar/webinar you may have attended. The purpose of this presentation is only to reiterate and underline the importance and the disproportionate collateral damage they can cause. Most of the mistakes may appear trivial to many and the need to correct them is simplistic. But it is good to remember that it is the small mistakes that lead to big problems, many of them preventable.
