About a year back I switched to topical phaco and I was having a good results, but in last OT I had a night mare of topical phaco.. A 70 year old patient was posted for topical phaco. He was completely co operative in pre operation, able to follow commands, not photo phobic. During per op I don't know in the middle of surgery(in quadrant removal), he is just became anxious, not opening eyes, moving eyes too much, that resulted in iris prolapse and PCR. ultimately I had to leave patient aphakic.
Now I am retrospect I am thinking by topical phaco what I am achieving?? It's more uncomfortable to surgeon. I think per op complications are high in patients. Visual acuity in one month follow up is the same in topical vs peribulbar phaco. Opinion of forum plz??
