A 92-year-old healthy man with just HTN as risk factor came with pain in the left maxillary area. His MRI brain attached. His hormonal profile and visual fields are normal. He has hemorrhagic infarct too in left parietal and temporal area. I think it is pituitary macroadenoma.

Pituitary macroadenoma to treat or not? Will you advise surgery in view of suprasellar extension? If so, with which approach?
