A 77 years old male known case of HTN, DM, hypothyroidism. History of diabetic nephropathy, also developed AF. Managed with Amiodarone infusion followed by tablet maintaining INR of 2 Episode of TIA recovered on its own without any neurological deficit. History of dyspnea at rest and orthopnea since 15 days, Managed with I.V. lasix.
Reported urine output reduced. Dialysis is done 4 times, just before the dialysis patient collapsed put on ventilator and extubated after 24 hrs. Presently patient on injection Meropenem, Polymyxin E and oxygen @4 lit /min Ranolazine, Atorvastatin, Salbutamol Ipratropium bromide nebulisation. Will pacemaker or implant defibrillator will help? How frequently dialysis to be done and any other treatments to be given?
