A Very interesting case I had in 1989. It was an era of initial sonography period. I had guest at home, sister of my wife who was a doctor herself. She was 1.5-month pregnancy and right iliac fossa pain. Total count etc suggested clinically appendicitis and treated conservative. Next day visiting Sonologist from near by city came with portable sonography machine and told there is uterus with gestation sac, so we continued appendicitis treatment.

But suddenly she went into shock after 8 hours and we did laprotomy, revealed ruptured right ectopic tubal pregnancy. With blood transfusion and surgery she was ok. We thought that diagnosis of uterine sac was the wrong or false sac. But post op after 15 days re sonography at higher Center revealed she had a uterine live pregnancy. She continued and delivered full term normally. Thus it was a case of twin pregnancy one tubal and one uterine.…
