A 4-year-old girl was brought to the surgical OPD with a left sided inguinal hernia. She was taken up for surgery and in the hernial sac an undescended testis was found, the pre-op USG confirmed both ovaries were present. Left sided orchidectomy with herniotomy was performed. She made an uneventful recovery.

HPE report confirmed left side testis. Her father was advised follow up with genetic and sex chromosomal studies. She had no other congenital anomalies.
