A 6 months old girl came with fever with early vomiting between 3 am to 9 am. She was given Syp cefixime for 3 days but not response later shifted to Amoxicillin/clavulanic acid but fever persisted. Her CBC, PS FOR MP, TYPHIDOT IS NEGATIVE, , BILIRUBIN, CRP WNL. Usg abdomen NAD..Urine report - Pus cell- 6-7, Albumin +nt,RBC 1-2, Bacteria present. Her urine c/s was sent.
She was started Inj ceftraixone inj amikacin. Pt improved very next day.Her Urine C/s sterile. She was continued with Inj ceftrixone and inj amikacin for 7 days. She had fever for initial two days of starting IV antibiotics.when IV antibiotic was stopped pt again developed fever and vomiting after 2-3 days.Urine was repeated it showed Albumin 2+, Pus cells 2-3,bacteria present. What to do now?
