A 23-year-old man was diagnosed with combined case of AIH and metabolic syndrome. Patient obese having lab tests reports ANA positive, IgG slightly raised, Vitamin D7, LFT ,deranged, uric acid raised, Dyslipidemia, IgE were very high. For AIH, Deflazacort with Azathioprine, other medications include Inj. vitamin D, allopurinol, the combination of Rosuvastatin and fenofibrate, UDCA, Silymarin with an advice of LSM to reduce body weight. The patient had strict compliance and reduce weight.

All lab tests came in normal range and ANA became negative after 3 months of treatment. Modifications in the administration of medications done except for Deflazacort and Azathioprine. In further 3 months, the patient took medications as advised but has poor compliance of LSM resulting in little weight gain.
