Dear All, It's shameful that we have lost a young life and I offer my deepest condolences to the family and the nation. It is also regrettable to see a majority of colleagues in comments above telling to refrain discussing this tragic death citing fears of press lurking in here. The fact is truth shall prevail and that is what we all need to demand and discuss and not just postmortem report only. Accountability must be fixed and whosoever has failed shall need top up training to prevent this in future and lessons need to be always learned by all, beginning from myself first.
We all on this forum also need to reflect and look inside us what and why we are discussing and what positive we can bring out of this tragic experience which has happened so as to prevent same happening again.
