A 56 yr old lady had a RTA one month back. Had cerebral edema with SDH with midline shift in CT head Along with communited # head humerus Lt hand. Also had an old infarct at rt MCA territory, so paralysed lt hand. Now presenting for head humerus , but still patient is irritable, and lt sided paralysed.

All routine investigations are WNL. Pt has tracheostomy  tube in situ. 1) Whether to go for surgery now or wait. 2) What should be adequate anaesthesia.
