A four-month male baby was brought with rapid breathing & fever since 5 Days. Condition worsened over the days. No prev.h/o cyanosis, difficulty in feeding oe neb. No h/o birth asphyxia Findings- Systolic murmur, liver 3 cm. Baby placed on ventilator but no improvement in saturation it remain 60% to 75%.
All things checked but no problem with tube or machine. Case treated as cardiac case started on dobutamine drip & later given milrinone as well. No improvement in saturation, once in a while baby picked up saturation to 90 % but only for short while. X-ray shows heart to be globular, aortic knuckle prominent ECG tracing are-given below.
