A 13-year-old 35 kg girl is having severe generalised pain abdomen with vomitings for last 10 days. Every year in April may since 2012, she experiences such episodes and is admitted for treatment. Loose watery stool for initial few days was present but even after recovery, the abdominal pain persists for weeks. Then she recovers completely and remains normal for the whole year. Now she is admitted to my clinic and following are the investigations: CBC:Hb 13. MCV MCH MCHC normal. Platelets 4.8 Lacs. WBC: 13K .N 74 .L 24.e 1.ps normal. ESR 35.
SGPT 51, SGOT .alp. N.Bilirubin 0.7 ALB 3.8. Globulin 4.1 A: G 0.95: 1. UR Routine and CLT N US ABD Lipase 71, Amylase 42. RBS 88. Didn't pass stool since admission so stool not invest. Ca 8.1, Na 142, K 4.5 Cr 0.7. Oral intake is still poor and induces vomiting. Pain abdomen and vomitings are still persisting though stool improved in a day.…
