A female of 45 years suffering from fever, vomiting since 2 days back. Past history: -   Fever since 15 days, mild cough, sometimes chills. She was hospitalized for it for 3 days.

Past Investigation showing mild pleural effusion and increase ESR. She was started on AKT. O/E today BP 130/90, RBS 124  Xray  Normal USG showing right pleural thickening with minimal pleural effusion   Kindly suggest differential diagnosis and management.
