An adult diabetic patient has thick milky White blood. Investigations: HbA1c 13.9gm %, cholesterol 1429mg%, Triglycerides 18000 mg% ,Pancreatic enzymes were elevated. Drug therapy: sitagliptan 100mg, dapagliflozin 10 mg, pantoprazole 40 mg daily but patient takes medication intermittently.

Patient has genetic marker for hypertriglceredemia. What should be further line of action?
