What is the treatment of this non responding Tinea corporis in an Alcoholic patient?

A 35 yrs old male, alcoholic, c/o itching over forearms with macules only, not responded with antihistamine, increased and spread, covered more area on forearms and abdomen, later spread to legs., given Itraconazole 200 mg per day, patient took for 15 days, then stopped for fast of Navraratra, restarted for 8 days now. But the disease is spreading like anything.
