A young patient about 21 years old came to emergency deptt on holi with history of poisoning. . He was not conscious with effortful respiration .

On examination his pupils were pin pointed , tachycardia was there , B.P was about 110 , pulse was good volume , we started oxygen , iv fluids , atropine , and naloxane was given but could not save that patient . What could if gone wrong ?
