Case presentation A 68-year-old male patient presented with a painless right testicular swelling. The swelling had increased progressively for the past six months. He also complained of nocturia and a slow urinary stream for the last two months. The patient was a well-controlled type 2 diabetic. Physical examination ·         Swelling of the right hemi-scrotum was noted, with the presence of a palpable hard mass on the right testicle.

·         The left testicle was normal. ·         The swelling was non-tender. Radiological investigations Ultrasonography of the scrotum revealed two hypoechoic and homogenous nodules in the right testis, measuring 10 x 8 mm and 16 x 10 mm, respectively.
