Case presentation A 19-year-old female presented with fatty tissue and drainage in the r ight lower quadrant of the abdomen (Cover image). The patient had undergone laparoscopic appendectomy for perforated appendicitis just two days ago. An aspiration drain was inserted into her Douglas space from the 5 mm trocar site , and she was discharged on the first postoperative day.
Physical examination An edematous, fatty, tubular, non-reducible structure was present at the port entrance site with associated serosal discharge . Investigations CT scan of the abdomen revealed an edematous, fatty, tubular structure , which herniated from the 5 mm trocar site in the right lower quadrant (Figure 1) Laparoscopic exploration revealed a herniated mass at the 5 mm trocar site (Figure 2).
