Case presentation: Mr. Mohan presented to the emergency department with serious respiratory complexities. Dr. Gupta had to insert a tube in his lungs to aid breathing; however, she failed to achieve optimum positioning of the tube. Mr. Mohan died as a result of oxygen being pumped into his stomach instead of his lungs.
