Case presentation An 81-year-old male with stage IV pancreatic ductal adenocarcinoma (PDAC) and liver metastases, undergoing chemotherapy, presented to the emergency department with worsening fatigue. Although his appearance was unwell, he remained conscious and maintained stable vital signs.
