A Child reported with bilateral scrotal swelling and hernia of the right side. The surgeon decided to operate. On phone, ordered staff nurse to give an injection of amoxicillin and clavulanic acid. After some time, adverse reaction developed which proved to be fatal. Observations:- After the operation, the surgeon did not provide Postoperative care. He saw patient only after the development of Adverse drug reaction. Surgeon passed orders on phone.

No documentation of orders of test dose before injection. The surgeon was asked to pay compensation of 10 lacs. Literature reveals that amoxicillin is less likely to cause anaphylactic shock than penicillin. Chances are more with injectable form than oral ones. In every 2 or 3 years, Fatal reaction with injection is on record. The test dose is must and it's not mandatory if the drug is given in infusion form. My view is unsatisfactory…
