A 18-year-old man was seen at health clinic a week ago complaining of pharyngitis, and now returns because he has noted discoloration of his urine. He is noted to have elevated blood pressure (178/110 mm Hg) and urinalysis reveals RBC casts, dysmorphic RBCs, and 1+ proteinuria.
