A 28-year-old woman was presented in ED with worsening abdominal pain and diarrhea over the last 2 days. She has a long history of ulcerative colitis. But she has been symptom-free for last 5 years. She mentioned, abdominal pain as general and associated with diarrhea that is increasing in frequency. She is unable to control her loose motion and noticed fresh blood with streaks of pus within the stool. She denied weight loss but mentioned feeling very lethargic. Examination She was tachycardia but normotensive.
The abdomen was distended but not peritonitic. She reported pain on deep palpation on the left upper quadrant. Blood test results suggested a degree of renal impairment and dehydration, with slightly elevated WBC count but normal Hb. Abdominal radiograph and CT of abdomen and pelvis were acquired.
