A 62-year-old man had past history of dilated cardiomyopathy and atrial fibrillation. He was put on warfarin and was followed up in the cardiac clinic. He presented to the emergency department due to the sudden onset of precordial chest pain after a bath at 7 o'clock in the afternoon. There was associated dull back pain. He was conscious but was in profound distress.
