52yrs old male, educated till 10th Class, premorbidy manning his family business of fertilizers and seeds, developed insidious onset apprehension restlessness and tremors of Rt Great toe at rest about 2 yrs ago. Gradually his apprehension increased and he started having difficulty in keeping the books in his business. He was taken to various Doctors at his hometown and nearby city and was prescribed medications. His tremors were benefited but he continued to have restlessness and apprehension. Now he cannot man his shop and has developed low mood and lack of interest. He has a tendency to fall while walking but has steady hands.
