A 58-year-old male was presented with bilateral proximal upper limb predominant muscle pain. It was initiated as a dull ache but became severe in nature within eight days. The pain was associated with swelling and restriction of movements at the shoulder joints. Episodes of polyuria and polydipsia were reported for the past two years. In addition, a family history of type 2 diabetes mellitus was reported in his mother and elder sibling.
