My pateint female 70 with history of Diabetes, CRF has started having orthopena since two days now. Her BS is around 200 PP, Urea 60 and Creatinine 4, Hb 6.8 gm/dl. She had had UTI and after Urine for CS a 10 days low dose Norfloxacin 200 is given. She responded well to the treatment for UTI. Anemia present, no oedema. She is on Inj. Erythropoetin 4000 IU twice a week. Clinidipine/Chlorthalidone 10 mg BID, Inj. Iron in 100 ml of NS given once. Tablet furosemide 1 OD., Cap.
Multivitamin and Doxyfyline and Acebrophylline 1 tab once a day. She has severe orthopnea. BP is maintained at 140 over 80. What are your thoughts on way forward for treatment ? Few suggested Digitalization (starting Digoxin) and 30 mg of Prednisolone ... Please, kindly suggest share your invaluable advice. Thank you for your time and suggestions. Thanks in advance.
