Dexmedetomidine in high doses, stimulates Alpha 1 receptor also, leading to High Blood pressure. This is observed in all routes of administration - IV infusion, intrathecal, Epidural, Brachial plexus block etc The rise in blood pressure varies in different individuals. Patients already using anti Hypertensive medications show less rise in blood pressure. Sometimes, an alarming rise in blood pressure is observed with Myocardial strain pattern in ECG, st elevations, Ectopics, etc.
Reverted to normal with reduction of blood pressure with NTG / sublingual Nifedipine, etc. Good monitoring, Prompt recognition, and understanding of the situation helps in handling the situation ideally. Share your opinion.
