A 38 year old male presented with chronic cough. On investigation, FBG - 168 mg %, PPBG - 286 mg %, HbA1c - 8 %, spot ACR found to be 400 and Blood Pressure - 136/86. Patient is kept on Metformin Glimepiride combination therapy.
For spot ACR or nephropathy, should we have to add ARB or ACE- I ? If yes, then what would be starting dose? If no, then how to counter nephropathy?
