A 72-year-old woman presented with pain in the left iliac fossa and no bowel disturbance. She had medical history of gastro-oesophageal reflux and mild dementia. On examination she was found tender in the left iliac fossa. The next day she had 4 episodes of watery diarrhoea and complained crampy abdominal pain. Examination On examination she had fever of 38.5°C and mild diffuse abdominal tenderness.

She has prominent bowel sounds.
