Case presentation A 72-year-old male with pleuritic chest pain and cellulitis in the left lower extremity presented to the emergency department. He was on ceftazidime and daptomycin treatment.

Past medical history Restless legs syndrome Morbid obesity and hyperlipidemia Sleep apnea Peripheral neuropathy Coronary artery disease L5-S1 spinal fusion Left knee, bilateral hip, and bilateral shoulder arthroplasties On daptomycin, vancomycin, and dicloxacillin for cellulitis Due to a peripherally inserted central catheter line infection, antibiotic treatment changed to daptomycin and ceftazidime. Examinations Left-sided chest pain with non-radiating pressure and exertional and pleuritic in nature Shortness of breath Electrocardiogram Normal sinus rhythm Right bundle branch block Comprehensive metabolic panel and troponins โ€“ Normal Blood count Eosinophilia โ€“ 7.0% Absolute eosinophil count โ€“โ€ฆ
