A 47-year-old male with type 1 diabetes mellitus presented with intense lower back pain for several weeks. He had six months of chronic night sweats, non-productive cough, and significant weight loss. He denied any headache, fever, joint pain, rashes or gait abnormalities. His physical examination revealed lower back tenderness and WBC count was 7,800/μL. Bilateral hilar lymph nodes with granuloma in the right middle lobe were detected in X-ray examination. Subcentimeter bilateral pulmonary nodules along with right hilar and middle mediastinal lymphadenopathy were observed in CT.

His HIV test was negative. No bladder numbness or incontinence was observed. MRI of lumbar spine has been shown in the image. A loss of 10-20% of vertebral height was observed.
