Case presentation A 90-year-old woman with itchy eczematous rashes in the extremities and trunk and swollen left leg ( Cover images a and c ) was admitted to the hospital on March 30 th , 2021. Medical history  Atrial fibrillation – Warfarin Hypertention – Bisoprolol, bendroflumethiazide, and amlodipine Primary biliary cirrhosis – Ursodeoxycholic acid Hypertension medications for more than ten years Unremarkable family medical history No previous skin diseases The onset of the rashes The rash started to appear after receiving the first dose of the COVID-19 mRNA vaccine, BNT162b2, on 29 December 2020. Her general practitioner suspected scabies and prescribed her moisturizers, hydrocortisone 1% cream, permethrin 5%, and an oral antihistamine.
