A 50 year old 91 kg man, presented with swelling of moderate grade of right foot and leg and mild swelling of left foot for last 5 days. The onset was with pain on the medial side of right thigh on the dorsal surface for last 10 days. The pain was very severe and walk with difficulty. Edema was pitting and few mobile palpable nodes in right inguinal region was noted which was non tender. The patient suffering diabetes from last 5 years and was on 2 tab of gluconorm daily. The fasting blood sugar was 135, uric acid 5.5 creatinine ratio (cr), 1.2, HB 12 total count 10500 n60 l 22 e10.

Cholesterol 185 tg 250 others N, Na, K, Ca, N. Three years back cr was 0.83 and weight 81 kg. So he gained 10 kg in 3 years. BP 138/96 in both upper limbs, uric acid was 7.7 TSH 4.8. So considering the rising cr will it be considered as early diabetic renopathy. Ur for micro albuminuria and TSH is awaited.…
