The basic difference between healthcare delivery systems internationally and nationally is of enforcement versus empowerment. The empowerment only lies with people who have the money, and the ones who can afford to buy their own insurance are empowered to get their healthcare wherever they like. The delivery system in India, for example for a laborer, is that of enforcement.
When an industrial laborer is working, some amount is deducted from his salary and he is forcibly told to go to a government hospital. He is enforced to get healthcare delivered from a particular hospital only and he is not empowered to go to any hospital that he wants. Source: Times of India
