An alleged medication safety lapse at a hospital in the Mumbai Metropolitan Region is being investigated after a 50-year-old patient died following treatment for dengue, falciparum malaria, and jaundice. The patient had initially presented with dizziness and vomiting before being diagnosed with the three conditions. An IV fluid was administered during treatment, following which his condition deteriorated, leading to a transfer to another hospital, where he required ventilator support in the ICU before his death.

The subsequent investigation found that the IV fluid had expired in July 2026. The municipal authority has suspended two staff nurses and a pharmacist for alleged dereliction of duty in connection with the incident.
