Intravenous methylprednisolone (IVMP) remains the cornerstone for managing active moderate to severe thyroid eye disease (TED), but the optimal dosing strategy has long been debated. A systematic review of 15 studies involving over 1,000 patients compared l ow (≤2.5 g), medium (2.5–4.5 g), and high (>4.5 g) dose regimens. Among these, the weekly medium-dose regimen (total 4.5 g over 12 weeks) demonstrated the best balance of efficacy and safety—improving Clinical Activity Score, reducing exophthalmos, and enhancing quality of life, with fewer adverse events compared to higher doses.

Despite heterogeneity among studies, evidence suggests this medium-dose protocol should be the preferred standard , though large-scale trials are needed to confirm its superiority and address recurrence management.
