A young nulliparous female with progressive abdominal distension and infertility was diagnosed with a large intramural Uterine Fibroid. Due to fertility preservation and non-availability of laparoscopic surgery, Open Myomectomy was planned. Intraoperatively, a Foley Catheter was placed circumferentially around the uterine isthmus and tightened to function as a temporary tourniquet, effectively reducing uterine blood flow. This allowed enucleation of the fibroid in a relatively bloodless field with precise dissection and preservation of myometrium.
Although intramyometrial Vasopressin is commonly used for hemostasis, it was avoided due to potential systemic complications. The surgery was completed with minimal blood loss, no transfusion requirement, and smooth postoperative recovery.
