Abstract Cholesterolosis is a benign condition of the gallbladder characterized by lipid accumulation within the mucosa. It is commonly seen in middle-aged females and often coexists with gallstone disease. We present a case highlighting classical gross and histopathological findings. Case Summary A middle-aged female presented with recurrent right upper quadrant pain and was diagnosed with symptomatic cholelithiasis. She underwent laparoscopic cholecystectomy, and the specimen was subjected to gross and histopathological examination.

Gross Findings The gallbladder specimen measured approximately 7–9 cm with mild wall thickening. On opening: Mucosa showed multiple yellow punctate deposits over a congested background, producing a typical “strawberry gallbladder” appearance. Bile-stained fluid with sludge was present.
