Beta-blockers (BB) comprise a basis of therapy of heart failure with reduced ejection fraction (HFrEF) in light of the findings of significant trials having shown that BB gives a long-term mortality reduction in these patients of up to 35%. Beta-blockers have been recommended by various guidelines for the management of patients with HFrEF. The current dilemmas include the choice of beta-blocker, its dose, the role of beta-blockers in light of newer therapies and the use of beta-blockers in patients with HFrEF with chronic kidney disease (CKD).
