Emerging evidence indicates that COVID-19 may leave behind subtle yet significant retinal microvascular changes, even in patients with mild disease. A prospective case-control study published in BMC Ophthalmology demonstrated a notable enlargement of the foveal avascular zone (FAZ) in recovered SARS-CoV-2 patients—particularly those who were never admitted to the ICU and those reporting long COVID-related fatigue—despite no major differences in macular vessel density. These findings suggest that FAZ alteration detected via macular OCTA could serve as a non-invasive biomarker for ongoing microvascular compromise and a potential predictor of long COVID fatigue, warranting closer ophthalmic surveillance in this patient population.
