In MRI with back pain, disc bulges found. Do you think surgery is the right algorithm? Most back pains these days are due to: Speed breakers (Automobiles )  Improper working posture, Obesity   Vitamin D deficiency Trauma etc Most of the people with back pain get cured with vitamin D injections. In my view, these days 50 % of back pains reduce with Vitamin D injections, without surgery.

I suggested this to many of my patients, and they got relieved. 25% need a good Physiotherapist, avoiding speed breakers,  Epidural Injections of Bupivacaine 0.5% and Butorphanol 4 mg along with Triamcinolone acetate 80- 160 mg. Only remaining 25%, after thorough assessment, require surgery. The algorithm is adequate vitamin D injections, Lifestyle modification, Epidural injection and then Surgery but not the reverse.
