About 40 days back someone referred this 6 yr muslim boy to me.The boy was sick for last 3 weeks and was on oral antibiotic since then advised by various pediatricians.He was extremely sick ,almost nil oral intake and fever shot upto 105 to 106 F at least twice a day and had running fever upto 103 to 104 F whole day.They had above investigations along with -ve malaria PS and optimal u report.I repeated OPtimal myself and then in lab which came -ve .SGPT was 140  and Bilr 2,direct 0.8. I institued Ceftriaxome,and Pipzo for 5 days but fever remained same.So pipzo was discontinued and Amikacin begun .The continous fever dropped upto 103 in another 5 days.But he still continued to have diurnal hypepyrexia till 105 F with intense rigors which was anyhow controlled by various antipyretics.Such diurnal peak forced me to repeat microscopy and after 3 hrs search of atleast 100  fields P…
