Although rare, common bile duct (CBD) injury is a devastating complication of laparoscopic cholecystectomy. While intraoperative indocyanine green (ICG) fluorescence cholangiography may improve biliary visualization, its role in preventing CBD injury remains uncertain. A cohort study analyzed data from 1,266,024 adults undergoing laparoscopic cholecystectomy to evaluate the association between ICG fluorescence cholangiography and surgical outcomes.
Of the study population, 164,695 patients (13%) underwent ICG fluorescence cholangiography. Within one year of surgery, ICG use was associated with lower rates of CBD injury (408 vs 659; relative risk [RR], 0.62), subsequent biliary intervention (7,387 vs 8,902; RR, 0.83), conversion to open surgery (657 vs 1,386; RR, 0.48), and nonbiliary complications at 30 days (11,939 vs 14,275; RR, 0.84).
