A 42-year-old man was admitted for acute promyelocytic leukemia. However, he developed acute respiratory failure after receiving all-trans retinoic acid for the treatment. CT of the chest had revealed multiple nodular lesions (1st image) which on biopsy showed invasive aspergillus species. The patient was treated with antifungal therapy and received mechanical ventilation. While his condition was improving, he had an episode of sudden and severe hemoptysis, followed by cardiac arrest.

He was resuscitated, but a severe neurologic deficit was developed. A brain CT was obtained (2nd Image). Identify the condition from attached CT image. *This case is from Docplexus editorial team for educative purpose only. Source: NEJM
