A 58-year-old woman presented with a 6-week history of intermittent passage of bright red blood from the rectum along with progressive fatigue and dyspnea. Her medical history was significant for recurrent episodes of spontaneous epistaxis since childhood, but she had never undergone full evaluation. Her mother also had frequent epistaxis. Examination Physical examination revealed telangiectasias on the labial mucosa (panel A) and nail folds (panel B). Laboratory results showed severe iron deficiency (Hb level 8.1g/dL).
