During left ventriculography in a 46-year-old male patient with hypertrophic cardiomyopathy, 20ml of contrast material was injected using a power injector at a rate of 10ml/sec through a multipurpose catheter. The left ventricle became opacified and ventricular tachycardia was developed. Immediate cardioversion was initiated. Initially, the catheter was free-floating. During the next beat, it was pulled in resulted in cannulation.
Contrast material was injected veins as indicated by black arrows with prompt visualization of veins indicated by arrowheads and white arrow.
