Mistakes do take place by any one of us during the course of our duty! Majority of them are unintentional and pardonable. But, some of them are due to sheer casualness and callous attitude. The first step is always to investigate as directed by medical or administrative authorities. Whenever I got a call to find lapses or omissions on the part of my colleagues, I subconsciously tried to give a clean chit or intentionally diluted the findings so that no harm is done to my brethren! Did I do the right thing?
